
Evidence of Insurability (EOI) is documentation
of a person's historical health events that is used by insurance companies
to determine whether a person meets the definition of good health.
An EOI form is required to increase employee life insurance coverage,
add long term disability or long term care coverage, and to add certain
dependents to medical coverage who were previously eligible, but not
enrolled during the employee’s initial 31-day benefit election
period.
