Email from the department containing the following:

  1. Account Title
  2. FMS Department ID
  3. Function
  4. If it is a new income generating account:

Budget Projection for the current year’s income

Provide a categorical budget for both revenue and expense pools.

This information is required for ALL requests to set up any kind of new Designated Fund account.

Business Plan

This is a management tool used to evaluate the financial feasibility of a proposal for an enhancement or new program. The business plan should detail revenue and expense for a specific time (3-5 years), and identify cash flow and recovery of initial investment.

Business plans are required for all requests to set up a new non-MSRDP Designated Fund account. They are not required for requests for new MSRDP Designated Fund accounts.

UBIT (Unrelated Business Income Tax) forms

These forms are available on line at http://www.utsystem.edu/bpm/bpmtable.htm

These forms are required for all requests to set up a new non-MSRDP Designated Fund account. They are not required for requests for new MSRDP Designated Fund accounts.

Review and Approval Process

Requests for new MSRDP Designated Fund accounts must be reviewed and approved by the leadership of the Practice Plan.

Requests for new non-MSRDP Designated Fund accounts must be reviewed and approved by the Chief Budget Officer.
