INSTRUCTIONS:
Please read this section carefully. Failure to follow all instructions may result in disqualification for award.

All applicants and recipients must be registered students or faculty in one of the schools or institutions of The University of Texas Health Science Center at Houston.

Your application must include the following materials and must be typed or printed legibly:

1. A current Curriculum Vitae

2. A one to three page statement including: 1) a brief background, 2) rationale for the international program and the need for support, 3) budget: how you would use the award 4) importance: what it would mean to you to be awarded, 5) benefit: how your department, school, or university will benefit from this award, 6) anticipated outcome and future plan

3. Supporting materials: recommendation letters (maximum three), prior achievements, affiliation, or interactions in international programs.

All application must be received before 5:00PM, Wednesday, March 1st, 2006. Applications can be mailed, faxed or e-mailed to:

The funding for this award is from benefactor donated money to the Office of International Programs and is subject to availability. International Programs Awards Committee will review the applications and select the awardees.
