Identity Verification: 
Candidates for Guest Accounts Who are Unable to Appear at a UTHSC-H Identity Verification Center

If a candidate is unable to appear at a University of Texas Health Science Center at Houston (UTHSC-H) Identity Verification Center, he or she must download the following  Microsoft Word documents and sign them before a notary public.

Once the "Guest Access Identify Form" has been notarized by the notary public, both forms must be mailed to the following postal address  DO NOT FAX FORMS!
