This information is provided for department heads and personnel with access to PeopleSoft to order dialup services from The University of Texas Health Science Center at Houston - Office of Academic Technology. This method should only be used for departments who will be paying for dialup accounts for their employees that are in the PeopleSoft system. For those indviduals who will be paying for their own dialup account, or are consultants or guests, please click here. Department heads and PeopleSoft HRMS users can apply for dialup service by requesting a dialup account (remote internet access) through System Data Resources (SDR). Departments will need to send the PeopleSoft Personal Information Change Request Web Form to the SDR team via e-mail at team_sdr@uth.tmc.edu. You must have a Digital ID to access the online form. To obtain a Digital ID, please apply here online.
PeopleSoft Personal Information Change Request Web Form New dialup users can be added simply by filling out the Personal Information Change Request web form and submitting this to the SDR team via e-mail at team_sdr@uth.tmc.edu. Please click here to access the web form. Please enter the user's e-mail address in the appropriate field. You must have a Digital ID to access the online form. To obtain a Digital ID, please apply here online. Personal Information Change Request Web Form Once the department funds are verified by Academic Technology and the FMS RQ is approved, the Academic Technology dialup administrator then activates the new dialup account.. The new dialup user(s) are then notified that they can now log on to their new dialup account by using their UTHSC-Houston User ID and Password (the same information they would use to log on to access their e-mail account). New dialup accounts are alotted 120 hrs./mo. usage with additional hours (above 120 hrs.) charged at $1/hr. New dialup users can then configure their home computers utilizing Windows
95, 98, ME, 2000 and XP.
