    You will find information about HIPAA forms and regulations here.

  5. Does CPHS offer training for research coordinators?
    Not formally, but we do hold monthly study coordinator lunches to discuss a variety of issues. Please contact Bridgit Solis at 713-704-4256 to get on the mailing list.

  6. How do I get the required Human Subjects Education Training?
    Here is a link to several training sources.

  7. Are there Continuing Education credits for Human Subjects Training?
    No, there are not.

  8. How long will the Education be valid?
    Most training programs certify for a two year period. CPHS has begun sending out reminder notices prior to Education expiration.

  9. How do I get a list of Committee members?
    The list of CPHS and AWC committee members is maintained by the Office of the Registrar at:

  10. What is the deadline for new CPHS applications?
    There is no deadline; protocols will be accepted at any time, and routed to the next available meeting.

  11. Where do I find the iRIS application?
    Logon to the iRIS application at http://iris.uth.tmc.edu.

  12. How do I get access to iRIS if I don’t work at UT?
    A guest account must be set up for non-UT faculty or staff. Here is link with information on the guest account process.

  13. What is your physical location?

  14. How do I get an AWC application form?
    The Animal Welfare Committee Application Form is in Microsoft Word format at this Web site: http://research.uth.tmc.edu/forms/protocol.doc

  15. What is the deadline for new AWC applications?
    New protocols are due by the 10th of each month, to be reviewed on the 4th Friday of each month.
