The Committee for the Protection of Human Subjects (CPHS) at the University of Texas Health Science Center at Houston (UTHSC-H) now serves as the Institutional Review Board (IRB) for the Memorial Hermann Healthcare System (MHHS). The CPHS must review and approve all research studies involving human subjects that utilize Memorial Hermann facilities or employees BEFORE its initiation.
MHHS and the Department of Health & Human Services (DHHS) have signed a Federal Wide Assurance Document (FWA #00000667) defining the obligation of the MHHS and CPHS to be certain that the rights and welfare of human subjects of research are protected, and the role that the CPHS plays in fulfilling this obligation. The following facilities, as components of the Memorial Hermann Healthcare System, are covered under the Federal Wide Assurance with DHHS:

If one of the other hospitals in the Memorial Hermann Hospital System will be used as a research site, it will be necessary for you to also fill out the Memorial Hermann Hospital System Application Form (in addition to the CPHS application form). Letters of approval to begin a research study must be received from both CPHS and the Memorial Hermann Hospital administration BEFORE research may begin in ANY hospital in the Memorial Hermann Hospital System.
Research that is conducted without approval by the CPHS is against federal regulations and institutional policy and is a violation of commitments made between the Memorial Hermann Healthcare System and UTHSC-H and its assurance of compliance document.
