Who needs a guest account?
Anyone who needs access to systems at UTHealth that is not currently a faculty, staff or student needs a UT Guest account. Examples of systems include Blackboard, iRIS and UT e-mail.
How do I obtain a guest account?
To become a guest of UTHealth, you need to be “sponsored”. If you are working with someone at UTHealth, they may be your logical sponsor. If you are a Memorial Hermann Hospital employee or M. D. Anderson employee doing research at UTHealth, someone in the Office of Research Support Committees can be your sponsor. (Contact Cynthia Edmonds at 713-500-7936 or Cynthia.L.Edmonds@uth.tmc.edu)
What information do I need to provide the sponsor need to get my account established?
The sponsor will need the following information to get your account established:
Name
Date of birth
City/State of birth
Country of birth
Country of citizenship
Driver's license number and state of issue OR passport number and country of issue
Employer name
Employer address
Employer phone
Valid email address
What steps do I need to take to obtain the guest account?
- Provide the sponsor with the information listed above.
- Be “validated” in one of the following ways:
- Present yourself before someone authorized to “validate” your identity.
- Present yourself before a notary with the proper paperwork necessary to obtain the guest account.
How do I actually get sponsored?
The sponsor needs to complete the Guest Sponsor Request form located here: https://apps.uth.tmc.edu/GuestAdmin/index.jsp
Who is authorized to “validate” my identity?
A list of people authorized to validate identity can be found on the following website: http://www.uth.tmc.edu/netcenter/middleware/digital-id/id-centers.html They are located throughout the Medical Center. If you are needing the account for research purposes only, you can also contact Barbara Legate at 713-500-3470 or Barbara.S.Legate@uth.tmc.edu for validation.
What if I can’t appear in person to be validated and I would like to use the “notary” option?
The forms that need to be notarized are located here: http://www.uth.tmc.edu/netcenter/security/notary-process.doc
One form is the “Guest Account Identity Verification Form” and the other is the Information Resources User Responsibilities & Accountability Acknowledgement Form”. Send the forms directly to Barbara Legate at 7000 Fannin, Suite 1021, Houston, TX 77030.
