Instructions and forms for Postdoctoral Appointments

All UTHealth Postdoctoral Appointments must be approved by the Director of the Office of Postdoctoral Affairs. This applies to all positions regardless of the source of funding. In order for new appointments, existing appointments, or salary adjustments to be considered for approval, you must submit an approval packet consisting of three documents. The table below will help you in assembling a complete and valid approval packet. All letters are to be printed on Department Letterhead. If you have any questions, please contact the Office of Postdoctoral Affairs.

The Appointment letter
OR
The Salary Adjustment Letter		 Choose only ONE of the following options:
  1. If the applicant is a US Citizen or US Permanent Resident, you will need to prepare an appointment letter. We have provided a form letter template to help you with this.

  2. If the applicant is NOT a US Citizen, You will need to sponsor the applicant for a visa. We have provided form letter templates to help you in preparing visa sponsor letters
  3. Please note there are three different H-1B letters. Please select the appropriate sponsorship status: H-1B Initial Sponsorship (for new UTH petitions) H-1B Transfer Sponsorship (from another institution) or H-1B Extension (of current UTH petitions)
  4. Please note there are three different J-1 letters. Please select the appropriate sponsorship status: J-1 Initial Sponsorship (for new UTH petitions) J-1 Transfer Sponsorship (from another institution) or J-1 Extension of Sponsorship (of current UTH petitions)
    ▪  The office of International Affairs is responsible for filing all visa applications. Instructions and information are available on the Office of International Affairs website.
  5. For changes in salary, you will need to complete a Salary Adjustment letter. We have provided a form letter template to help you with this.

The Postdoctoral Appointment Request Worksheet		 Download and complete the Postdoctoral Appointment Request Worksheet. Detailed instructions are included on the form itself. Incomplete forms may be returned delaying the processing of the appointment.
Document #3
A current Curriculum Vitae		 The CV must be current and indicate the date the doctoral/terminal degree was granted. Contact the Office of Postdoctoral Affairs if you have questions regarding the requirements.
Submit your approval packet Submit your approval packet containing the above three documents to the Office of Postdoctoral Affairs at MSE R256. Please note - Department contact name and phone number must accompany all approval packets. It is suggested that you contact The Office of Postdoctoral Affairs prior to submitting documents.
