Photopenic defects with persistent cerebral perfusion and metabolism.
41 y/o male with history of longstanding hypertension and resultant intracranial hemorrhage on the right.

A. Asymmetric but persistent flow is identified on ECD perfusion study, left greater than right.

B. Immediate and delayed static images demonstrate a photopenic defect in the right hemisphere (arrows). Rim of activity visualized around the photopenic defect on the static images indicates persistent metabolic function of the right hemisphere.

C. A large right hemispheric bleed noted on CT images of the brain corresponds to the photopenic defect seen on the scintigraphic study.
