Extracranial versus intracranial leakage of activity via skull defect in the presence of brain death.

24-year-old male status post GSW to the head.

A. DTPA perfusion study demonstrates no flow in the distribution of the bilateral ACA's and MCA's, compatible with brain death. Evaluation of brain metabolism is not possible with DTPA.

B. Collection of activity is noted extracranially on the right, which may represent active bleed into a skull wound (arrow).

C. Axial CT images demonstrate an extensive bullet wound on the right (arrow)
