Delivery of samples to the QGCL must be accompanied by a sample submission form. There are different forms for nucleic acids vs proteins (MSD & AlphaLISA). The forms must be filled out completely. If not, the samples will have to be resubmitted with a completed form.

The sample submission forms will be migrating to the CCTS LIMS (Genologics). At that time, the sample submission forms will be filled out on-line prior to bringing the samples to the QGCL.
