The Acres Homes Center was the first clinic in HCHD to be utilized as a model Family Medicine Residency Clinic. Located nine miles north of Houston's central business district, Acres Homes is one of the five HCHD clinics staffed by the Department of Family Medicin faculty physicians. It is located in a federally designated Health Professional shortage area (HPSA) and provides care for primarily underserved, indigent or Medicaid patients. The multicultural patient population at Acres Homes is predominatly African-American with a significant Hispanic population and some Asian and European patients as well as a sizable geriatric population. Residents are responsible for their own panel of patients from this community with Family Medicine faculty available for clinical consultation. There is an up-to-date library that provides current reference materials as well as on-line computer search capabilities. This site also includes an in-house pharmacy and ancillary services for patients.

The Aldine Community Health Center, built in 1996, was specifically designed as a teaching site and has an in-house pharmacy, general radiology services, dentistry services, WIC services, as well as a full-time social worker. The Aldine Center is located in a federally designated Health Professional shortage Area (HPSA). Similar to Acres Homes, residents are responsible for their own panel of patients from a primarily indigent community. Family Medicine faculty are always available for clinical consultation. An up-to-date Family Medicine library provides current reference materials as well as on-line computer search capabilities.

Approximately 33% of the required rotations are at LBJ. This well-equipped public hospital was opened in July 1989, and it is here that our residents gain invaluable experience while serving a very needy community. All five HCHD clinics in our Urban Program in the Department admit their patients to the Family Medicine Service at LBJ.
