Hardeep Singh, MD, MPH
is an Assistant Professor of Medicine at the Michael E. DeBakey Veterans Affairs Medical Center and Baylor College of Medicine and Chief of the Health Policy and Quality Program at the Houston VA HSR&D Center of Excellence. He is the Director of the Houston VA Patient Safety Center of Inquiry, funded by the VA National Center for Patient Safety. This center uses a multidisciplinary approach including human factors engineering to improve systems of communication through the VA's electronic medical record (CPRS). Additionally, he is funded through several federal grants from the AHRQ and NIH for patient safety related research and implementation activities. Dr. Singh has significant expertise in the study of medical errors, patient safety, and health care quality, including translating aviation safety practices to health care and evaluating system and provider factors that lead to errors in health care. He is a nationally recognized expert in improving patient safety through electronic medical records and on using health information technology to identify and reduce medical errors and has participated in several national and international expert committees and invited conferences.
