Health information technology (IT) has the potential to facilitate communication and coordination of care between several disciplines and settings that constitute ambulatory care. In cancer care, the concept of patient navigation has been proposed to minimize barriers in the movement of patients through the complex cancer care system and thereby help ensure timely diagnosis and treatment. But a key question remains: among thousands of patients, how does a health care system efficiently identify those likely to be experiencing diagnostic and treatment delays and correct those delays? This study will use IT based monitoring techniques to identify patients likely to have delays in cancer diagnosis or treatment. A cluster RCT will be used to test the effectiveness of this intervention.
