Using Electronic Records to Detect and Learn from Ambulatory Diagnostic Errors

Diagnostic errors are likely the most common, expensive, and harmful type of error in primary care. This study will develop and test an electronic trigger tool to identify diagnostic errors by screening data available in electronic health records. This will enable primary care practices to identify, learn from, and prevent these errors.
