Project Description
The goal of this grant is to improve the health of pediatric patients with complex conditions by:
- Improving our understanding of the critical error points that introduce risk during transition from the intensive care to ambulatory environment
- Designing an efficient and generalizable model of transition between these settings that is based on system changes to mitigate these sources of error
