Safety Attitudes and Safety Climate Questionnaire

The Safety Attitudes Questionnaire was developed Bryan Sexton, Eric Thomas, and Bob Helmreich with funding from the Robert Wood Johnson Foundation and Agency for Healthcare Research and Quality.  The standard citation for the survey is:
