Do you have a child with special needs?

Have you had trouble finding a pediatrician who will treat your child with special needs? With more than one million children in the United States suffering with chronic illnesses and disabilities, services to these children and their families are often fragmented, uncoordinated, and difficult to obtain. The Department of Pediatrics at The University of Texas Medical School at Houston recognizes these barriers to care and is proud to offer CHOSEN Health Care for Children with Special Needs. The CHOSEN program serves as a medical home for your child, providing a primary care pediatrician, a social worker, and a nurse for your child from birth to 21 years old.

What the CHOSEN program provides for you and your child

  • Family-centered and culturally-sensitive general pediatric care for annual well-child visits and sick visits;
  • Management by a primary care physician who has expertise in complex chronic conditions;
  • Appropriate referrals to and consultations with specialists;
  • Coordination with home health services, outpatient therapy agencies, and medical equipment companies;
  • Social work, Medicaid case management, and counseling services;
  • Palliative and comfort care services;
  • Coordinated care with the inpatient services at Children’s Memorial Hermann Hospital, LBJ General Hospital, and HealthBridge Children’s Hospital.

Your child will qualify for CHOSEN if he/she has:

  • Cerebral palsy;
  • Traumatic brain injury or spinal cord injury;
  • Neurological or developmental disorders, including epilepsy, spina bifida, and autism;
  • Chronic lung disease;
  • Congenital heart disease;
  • Orthopedic impairments;
  • Genetic syndromes or chromosomal abnormalities;
  • Requirements for gastrostomy, tracheostomy, ventilator, and/or total parenteral nutrition;
  • Complications of prematurity; or
  • Any other chronic illness or disabling condition.

Referrals

Referrals and requests for information may be made by families, foster care families, professionals, and agencies.
