Hospitalization/Dental/Disability/Life/Professional Liability

Health Insurance

The Medical Foundation pays for the employee-only portion of TMA Plan I, $300,000 deductible.  Employee portion paid by the Medical Foundation, HMO, $124.96 PPO, $152.98.  Coverage for dependents is paid by monthly payroll deductions.

If you elect: HMO PPO
To add your spouse, you pay $123.31 $151.33
To add your child(ren), you pay: 110.98 137.82
To add your family, you pay: $271.28 $335.64

              

Dental Insurance

The Medical Foundation pays for the employee-only portion of TMA Dental Insurance, $19.29. Coverage for dependents is paid by monthly payroll deductions.

If you elect:
To add your spouse, you pay $10.67
To add your child(ren), you pay: $17.37
To add your family, you pay: $29.07

     

Disability Insurance

The Medical Foundation pays the premium for long-term disability insurance. Under this plan, $1,000 per month is payable to you in the event you become disabled in accordance with the provisions of the plan.

  Group Term Life Insurance

The Medical Foundation pays for $100,000 coverage, $6.00. Coverage for your dependents is paid by monthly payroll deductions. Spouse coverage is $10,000. Dependent children may be covered for $5,000 each.

If you elect:
To add your spouse, you pay $0.99
To add your child(ren), you pay: $0.99
To add your family, you pay: $0.99

 

Accidental Death and Dismemberment Insurance

You are eligible to purchase from $10,000-1,000,000 in coverage for yourself through monthly payroll deductions.

To cover yourself for PGY 1 and the first $10,000, you pay: $8.30
For additional coverage in $10,000 increments, you pay .30 per $10,000
