When ordered by a physician, dental care is provided for
patients at UTHCPC.
Procedure:
Dental care shall be available to adult and child
patients on an emergency and non-emergency basis.
A physician's order for dental consult is
entered into the computer by nursing/medical staff.

Phone # 713-500-4246 or fax 713-500-0403 for appointments
or Ben Taub Center after hours and on
weekends for emergencies
The Application for Dental Treatment form, the Medical
Questionnaire, and the UTHCPC Request for Consultation shall be completed by
requesting staff and signed by the adult patient, parent (if child) or
guardian. These forms shall be retained
in the chart and sent with the patient to the Dental Clinic.
The Dental School must be notified prior to sending the patient to the Dental School. NOTE:
Patient’s appearing without prior notice will not be seen.
When consultation has been completed,
the UTHCPC consultation form shall be returned.
