This table describes the process for requesting and moving furniture and equipment:

The employee includes the following information in a memo or email to Support Services at least three working days prior to the expected move:

Requested move date

Current location

New location

Items to move

Key or lock changes

The employee completes the Move Request and submits it to Support Services at 1D48 prior to the move.

The employee removes files, books, and desk items from the furniture and properly boxes it.

Note: S/he must supply his/her own boxes.

Person requesting and/or responsible for the move directs placement of the furniture/equipment in the new location as the move takes place.
