Infection control practitioner
Qualifications
: A staff member with documented training and experience in infectious disease prevention and control.

Attendance Goal: 85% of committee members at each meeting

Responsibilities

The Infection Control Committee has the following responsibilities:

General

Define the authority of the Infection Control Plan in writing

Obtain approval from the administration and medical staff for the Infection Control Plan

Determine and describe, in the form of policies and procedures, the type of surveillance necessary to prevent and/or control infections in hospital staff and patients

Determine policies, procedures, materials, and/or equipment necessary to mitigate disease transmission within the facility

Monitor the results of surveillance

Report all infection control concerns to the Performance Improvement Council

Reporting mechanisms

The Infection Control Committee determines the reporting mechanisms for the following:

Patient identification and location

The following dates:

Admission

Onset of infection

The following diagnosis information:

Type of infection

Cultures taken

Findings

Antibiotics administered

Identification of attending physician

Designate authority

The Infection Control Committee designates the individual(s) responsible for the following actions:

Surveillance activities:

Collecting data

Analyzing data

Reaching conclusions regarding surveillance data

Control activities:

Taking action

Assessing action taken

Reviewing Infection Control policies and procedures annually, or more frequently as needed.
