The following are the disease reporting categories:

Diseases reported immediately by telephone

Diseases reported by numerical totals

Responsibilities

This table lists the Infection Control Practitioner's disease reporting responsibilities:

Responsibility	Description
List maintenance	Maintains the following current lists: Reportable diseases as published by the Texas Department of Health Diseases reported by the contracting laboratory service
Information collection	Collects disease information for reporting from: Patient records Laboratory reports 24-hour nursing reports Anti-infective order reports
Reporting	Reports to the office specified in the current Reportable Diseases list by telephone or mail as required
