SME: Patients Relations Manager
UTHCPC provides a Bill of Rights for child, adolescent, and adult patients or
their legal representatives upon admission. UTHCPC Staff assists patients, throughout
their hospitalization, with exercising their rights in a manner that respects
the rights of other
individuals.
Upon admission, the patient/representative receives different rights documents depending on his/her age:

![]() | A minor over eight years old receives Teen's Bill of Rights and Responsibilities. |
![]() | A minor under eight years of age receives Children's Bill of Rights/a story called The Little Dinosaur Named Wilbur available from the Patient Relations office. |

This table describes how UTHCPC staff communicates patient rights to the patient:

Admission staff documents the receipt of the rights documents in the Patient Rights Signature form. |

Nursing staff fills the rest of the form. |

The nurse documents the patient's review of the rights for the first time. |

The nurse documents in the form if the patient is unwilling or unable to sign the form to confirm his/her understanding of his/her rights. |

The nurse puts the form in the patient record after it is complete. |
7 |
Within 24 hours of admission and on a 24-hour basis, the nurse explains the patient rights until the patient understands it. |
8 |
The nurse documents all attempts to explain the rights in the patient record noting any reasons for the patient's lack of understanding. |
