Patient Admissions

Patient admission to UTHCPC is as follows:

Involuntary admission

Voluntary admission

Direct admissions

Patient referrals for direct admission to UTHCPC must be as follows:

Approved by the Executive Director or Medical Director

Screened and referred by a licensed physician

Meet criteria for a UT admission

If approved, the Executive Director or Medical Director will notify admission staff of the anticipated direct admission

Patient documentation and material

The following documents and material accompanies the patient to the nursing units:

Original of legal paperwork (MHW, EDO, etc.)

Acknowledgement of receipt of patient rights and patient handbook

Consent to treatment/various other consents and acknowledgements for treatment

Copy of patient's valuable/clothing list

Copy of Advance Directive request

Transferring hospital or MHMRA intake packet (if screened by MHMRA Services, e.g. clinics, NPC, etc.)

One patient photograph

Addressograph Card

HISD School Packet & Consent for Transportation (Child/Adolescent only)

Evidence of request for most recent medical record from Health Information Management (HIM)

Administrative and Medical Clearance form

Memorandum of Transfer

HISD forms required

HISD requires that the Patient Registration Department only initiates the following forms as provided by HISD:

Student check-out sheets for Elementary and Secondary school (HISD personnel complete it)

HISD Emotionally Disturbed Documentation form (a psychologist completes it)

Report of Physical Examination (a physician completes it)

Patient properties and valuables

Patient Registration staff do the following with the patient's valuables and properties:

Explain UTHCPC policies concerning patient valuables and excess property
Sending valuables
: Patients are encouraged to send valuables, excess money, medications, and personal items home with family members upon admission.

Account for all patient money upon admission as declared by patient

Tag and store excess patient property (Room 1A-32)

Forward all medications to the designated nursing unit upon completion of the interview

Form signing requirements

The following are required when the patient signs the forms:

The form must be accurately completed and signed in the presence of a witness.

Patient refusal to sign the form must be noted on the form and witnessed by the Patient Registration staff.

Involuntary

This table describes the process of admitting an involuntary patient:

Upon patient arrival, Harris County Constables open the Side Patient Entrance door and notify the Patient Registration office.

Patient Registration staff notify the designated nursing unit of the patient arrival and then checks for the accuracy of all the legal paperwork the constable has submitted.

Nursing staff conduct a patient body search prior to the admission interview.

The patient is admitted into the Allegra system and assigned a medical record number. Patient Registration staff do the following:

Make the patient demographic face sheets, addressograph card, and I.D. Bracelet

Place the addressograph information on admission forms

Patient Registration staff then proceed with the patient interview by explaining and asking the patient to sign the appropriate forms.

Patient Registration staff document on all forms the name of the person who will sign on the forms if other than the patient.

Example: Parents, legal guardian, conservator

If the patient consents, Patient Registration staff take one photograph and attach it to the documents on the unit.

Patient Registration staff forward all the documents and material to the nursing units.

Voluntary patient consent

The Consent to Admission and Treatment form must be signed for the voluntary admission of adults, children, and adolescents using the following guidelines:

The patient is 16 or 17 years of age

The patient may sign herself/himself in

The patient's parent or guardian may sign the patient in if the patient is unwilling to come in voluntarily

The patient under 16 years of age

One of the following signs:

The parents

Legal guardian

Managing conservator

The guardian or managing conservator is an employee or agent of the State of Texas or a political subdivision and the patient is a minor

Both of the following sign:

The patient

The guardian or conservator

Documentation required

The legal guardians are required to submit the following documentation for a child/adolescent under 16 years of age:

IF...

THEN staff must obtain...

The natural parents are married

The child's birth certificate

The natural parents are divorced

A copy of the divorce decree and birth certificate

The parent is not the managing conservator of the child

A notarized statement from the managing conservator

The patient is adopted

A copy of the adoption papers

The patient is in the custody of Harris County Children's Protective Services or another agency

A copy of the custody papers

Admitting a voluntary patient

This table describes the process of admitting a voluntary patient:

The reception desk notifies the Patient Registration office of the patient arrival.

Unit nursing staff notify MD of patient's arrival and need for an admit order. Triage Resident writes orders for admission and a brief progress note.

Patient Registration staff explain the Consent for Admission and Treatment form and asks the patient to sign.

4

Patient Registration staff follow steps 3-9 mentioned in the involuntary admission.

Direct admissions

Patient Registration staff follow the same steps mentioned in the involuntary admission.
