When determined
The admitting physician orders the Social Service Clinician to assess the patient and involve in group therapy per criteria as appropriate. This order defaults to Level I group assignment for every patient. Once the patient attends the first group or the psychosocial is completed, it is determined by the group therapist a nd/or the case manager if the patient is ready to be involved in Level II groups. Individual group goals are established upon the first contact with the patient in a group setting.
Assessment completion
Recreation Therapy services are coordinated through the Social Service department, drawing from data collected in the psychosocial, group observation, and chart review. The therapist will complete an assessment summary note follwoing the first group the patient attends. The assessment summary note is entered in the Sunrise system following the first group and is entered within 24 hours of the group.
