Social Services Group Process

Referrals for groups

The admitting physician orders that the patient be assessed "assessed and involved in group therapy per criteria, as appropriate". This order defaults to Level I group assignment for every patient. Once the patient attends the first group, the group therapists may recommend to the Social Service Clinician (SSC) a change to Level II groups. When an SSC completes a psychosocial assessment, the SSC will assign Level II groups when appropriate. The group facilitator develops with the patient in group specific goals upon the first contact with patient.

Documentation

Documentation is in the social service progress notes section of Sunrise. Group progress notes are comprised of the following:

Data - a report of the patient's measurable behaviors and through facilitator’s observations and/or patient report.

Assessment – a clinical evaluation of the patient's cognitive, affective, and behavioral progress toward the treatment goal.

Plan - steps that will be taken by the therapist to revise or continue the patient goal

Documentation of educational/didactic groups is completed in the social service educational note section of Sunrise. Content of the note includes attendance, name of group, and evaluation of patient’s level of understanding of the material presented.

Staffing for groups

The clinicians provide psychotherapy and educational groups for patients. Coverage is accomplished by staff being:

Assigned to specific units and group topics.

Rotating weekend coverage to facilitate hospital-wide groups for adults and unit-based groups for children and adolescents

All group psychotherapy is conducted by masters level clinicians that are licensed by the state of Texas (LMSW, LCSW, LPC). Spirituality, music, and recreation therapy is delivered by staff that is Certified in their specific discipline.
