Controlled Drug Number/DEA #

Whenever a physician writes a prescription for a controlled drug (other than Class II), s/he needs a DEA number on the prescription. .

Where/when to obtain

A physician should go to the Pharmacy to request a suffix to the HCPC DEA number prior to writing prescriptions for controlled drugs. The Pharmacy issues a number once the physician has signed an identification log.

Caution

The physician must keep the number in a safe place and use it on all prescriptions for controlled drugs written at UTHCPC.

Other prescriptions

The physician must not use a DEA suffix for the following prescriptions:

Issued at another institution for prescriptions written at UTHCPC and vice versa.

Do not use this number for Class II prescriptions. Class II prescriptions must be written on a DPS issued prescription pads by the personal holder of the DPS pad.
