UT-HCPC has a closed formulary system. The physician must consult the Pharmacy when s/he is in doubt about whether a drug is on the formulary at UT-HCPC. Physicians are expected to use formulary medications unless there is a justified and overriding need to prescribe a non-formulary medication..
If a physician wishes to prescribe a medication that is not on the formulary, s/he must first document failure to formulary agents before requesting approval for use of a non-formulary medication. The pharmacist is available for consultation to recommend a formulary agent.
Staff physicians can recommend that the Pharmacy and Therapeutic Committee consider additions to the UT-HCPC Formulary.
Alternative drugs might be available when a physician orders a drug that is not on the UTHCPC formulary. The pharmacist recommends any alternative drug that is available on the formulary.
The physician must do the following if s/he does not accept the alternative drug:
![]() | Completes the Non-Formulary Request form that the Pharmacy sends to him/her |
![]() | The attending physician must sign the request form. The Attending physician forwards toe request form to the Medical Director for his review and approval if appropriate. |
![]() | The completed form is then returned to Pharmacy for dispensing. |
The Pharmacy returns any incomplete form to the physician for completion.
