Look Alike Sound Alike Medication Policy

Introduction: In response to the Joint Commission's National Patient Safety Goal of Improving the safety of using medications, a list of LOOK- ALIKE SOUND-ALIKE medications used at HCPC has been developed.

The Medical Staff through the Pharmacy and Therapeutics (P&T) Committee has determined the list of drugs designated as look­ alike sound- alike medications.This list will be reviewed annually by the P&T Committee.

All staff involved in the medication use process shall be educated on these look-alike-sound-alike drugs. HWE will assist in this education process.

A poster of this list of Look Alike Sound Alike drugs shall be maintained on each nursing unit.

This is a list of the brand names and generic names of drugs that look alike and sound alike. Each drug name has been cross-referenced.
