Subacute Unit Medical/Dental and X-Ray Procedures

Policy:

It is the policy of UTHCPC that all medical appointments for Subacute patients will be coordinated with JDC so that there is adequate transfer of information and adequate staffing provided to ensure safe transport of patients to and from medical appointments

   
Procedure:

1. Emergent Care:

1a. Emergent Medical Care:

Per the agreement with JDC, emergency care is provided by Ben Taub emergency room in cases of 911.

Transportation/escort: Hospital staff members will accompany patient. If the time in the ER exceeds 4 hrs, JDC will send staff member to the ER.

Communication of information: Patient instruction sheet is returned by the UTHCPC staff member and given to the nurse.

1b. Emergent Dental Care:

Is provided through the UT Dental clinic.

Transportation/escort: Hospital staff members will accompany patient.

Communication of information: Care instructions are provided by the clinic to the staff member accompanying patient. UTHCPC staff member gives the unit nurse the copy of the instructions upon return.

1c. Emergent X-rays:

Are provided through Herman Hospital Radiology dept.

Transportation: Hospital staff members will accompany patient. After hours escorT support will be requested through JDC if the wait time is expected to exceed 4 hours.

Communication of information: Initial results are called back to unit nurse.

2. Nonemergent Care:

2a. Nonemergent routine preventative and follow up medical care is provided through the JDC clinic. The clinic is staffed by UT Pediatricians and residents.

2b. Nonemergent Dental Care: Routine dental screening is provided through dental services provided at the JDC clinic.

2c. Nonemergent X-rays: JDC clinic physicians may order X-rays as part of their required screening services. The quality of the X-ray is the responsibility of JDC. While x-rays performed at JDC may be exchanged for information purposes, UTHCPC physicians are not to use them for treatment decisions.

Transportation: Transportation for nonemergent care (dental and medical) is provided by JDC staff members.

Communication of information: Follow up recommendations are carried by the JDC staff member accompanying patient. Upon return from appointment, the follow up recommendations are given to the Subacute nurse in charge. The nurse will inform Subacute Attending of the receipt of the follow up documentation.
