Drug-Nutrient Interactions Policy

POLICY:

1.
Prior to discharge, patients will receive general instructions about avoiding drug-nutrient interactions and more specific education about the potential interactions between certain foods and selected medications that may have clinically significant adverse outcomes.

 

     
2.
Medications for more detailed education will be selected by the Departments of Pharmacy and Clinical Nutrition on the basis of scientific evidence of significant adverse events due to the interactions of medications and nutrients.  

PURPOSE:

1.
To educate the patient and other caregivers about the potential interactions between certain medications and foods they will be taking to promote the safe use of medications    

 

RESPONSIBILITIES:

 

1.
Pharmacy will run a report daily to screen for selected drugs upon admission and notify Clinical Nutrition via established policy.
2.
Nursing is responsible for the education of patients receiving calcium channel blockers upon discharge.
4.
Clinical Nutrition is responsible for the drug-nutrient education for patients being discharged on the following medications.
  a. Isoniazid
  b. Monoamine oxidase inhibitors (MAOI)
  c. Warfarin
  d. Certain Antibiotics
5.
The Pharmacy will prepare the monographs in coordination with the Patient Education Committee and other appropriate groups. Pharmacists may provide medication use counseling that includes drug-nutrient interaction information as necessary.

 

PROCEDURES:

 

1.
Verbal and written information will be provided to the patient or the caregiver of a patient being discharged on the designated medication by a nurse, clinical dietitian, or pharmacist.
2.
The Pharmacy will provide a list of patients on the targeted medications to the Clinical Nutrition Department on a daily basis.
3.
Patients being discharged on warfarin may be referred to the Anticoagulation Clinic for follow-up by the appropriate caregivers.
5.
Clinical Nutrition will educate the patient and/or the patient’s caregivers receiving the identified medications from the list of patients identified by Pharmacy.
6.
Drug information master sheets will be maintained by the Pharmacy and distributed to the patient care areas and Clinical Nutrition .
7..
Documentation will be completed by all disciplines on the Patient Education Assessment and Documentation Form. Dietitians will also document drug-nutrient education with the patient/caregiver in the progress notes of the medical record using the standard nutrition education form. This form documents the written and verbal learning responses obtained.
