Signature Authorization Commitment/Transfer Documentation

In order to deliver quality patient care, it is important to review documents pertaining to patient commitment and transfer for proper completion. UTHCPC requires that an administrator or his/her designee review and sign these documents.

Authorized Personnel

The primary responsibility of documentation review and authorization belongs to any one of the following:

The nurse manager or designee

The nursing supervisor

Any administrator

Documentation

The following documents are used in the patient's official record. The following documents should be reviewed, signed and authorized:

Termination of Court Ordered In-Patient Mental Health Services

Letter of Retraction for Release from Voluntary admission

Request for a Patient Transfer to a State Facility

Request for Release from Voluntary Admission

Administrative Request for Court-Ordered Services

Memorandum of Transfer (both in and out)

Form Distribution

Additional copies of all forms must be delivered according to the following:

The court

The patient

The unit (placed in patients medical record by nurse or designee)

Related standard

Related to regulatory standards
