In order to deliver quality patient care, it is important to review documents pertaining to patient commitment and transfer for proper completion. UTHCPC requires that an administrator or his/her designee review and sign these documents.
The primary responsibility of documentation review and authorization belongs to any one of the following:

![]() | Any administrator |
The following documents are used in the patient's official record and need review and authorization:
![]() | Termination of Court Ordered In-Patient Mental Health Services |

![]() | Memorandum of Transfer (both in and out) |
Copies of forms must be delivered according to the distribution instructions to the following:

![]() | The patient |
![]() | The unit so the nursing staff places them in the medical record |
