Physicians must notify the court of treatment recommendations so that the Court
can make an accurate assessment of the need for commitment.
The attending physician completes and signs a Certificate of Least Restrictive Alternative for:

![]() | Patients being considered for modification from out-patient to in-patient status |
The MHMRA/UTHCPC Court Liaison or designee distributes copies as follows:

![]() | Copy to the Court |
![]() | Copy to patient's medical record |
![]() | Copy to Court Liaison's file |
Revision of treatment recommendation is as follows:
![]() | The attending physician can reevaluate and revise the treatment recommendation anytime preceding the patient's final court hearing. |
![]() | The attending physician must complete and sign a revised Certificate of Least Restrictive Alternative form noting in the Comments section the reason(s) for the revision. |
![]() | The attending physician submits the revised form to the MHMRA/UTHCPC Court Liaison's office in time to be filed with the County Clerk's office at least 24 hours prior to the patient's final hearing. |
The MHMRA Court Liaison or designee distributes copies of the revised form same as the original.
If a change in treatment becomes necessary the day before court or in an emergency situation, the attending physician must contact the MHMRA court physician.
