Probationary employees

This table describes the review process for probationary employees:

Stage

Description

Department manager completes 60-day evaluation form for each applicable probationary employee and places the form in the employee personnel file.

2

Department manager completes 120-day evaluation form for each applicable probationary employee and places the form in the employee personnel file.

Note: Department manager has the option to extend probation to 180 days.

Review notification

This table describes the process for reminding department managers when they have reviews due:

Produces an annual memo every September indicating the month in which the evaluation is due for each staff member

Sends the annual memo to all department managers

Produces a monthly reminder memo listing evaluations due for that month

4

Sends the reminder memo to each department manager who has an evaluation due

Review and documentation

This table describes the performance evaluation review and documentation process:

Description

Department manager has employee complete a self-evaluation form.

Department manager completes the performance evaluation.

Department manager and employee discuss the employee's:

Success in meeting performance expectations

Contributions to the organization

Performance improvement needs

Department manager and employee determine the following:

Update performance expectations

Identify development needs/goals

Department manager completes the Evaluation Confirmation form.

Information to include: Overall evaluation score and date

Department manager and employee review employee's development plan and job description to determine if they need to make any changes based on the items discussed/determined in the performance evaluation.

Department Manager sends the completed performance evaluation form to Personnel Systems Management (PSM) to be filed in the personnel file. Completed forms are to be received by PSM by the fifth day of the following month.

Notes:

If the fifth falls on a weekend or holiday, the department manager must send the form to PSM by the previous work day.

Departmental managers can request an extension on the month due up to 1 month based on extenuating circumstances. Requests should be made to the director of PSM in advance of the month due. Further extensions can be requested for employees on FMLA, medical leave, WCI or personal leave of absence.

Evaluation delinquency notification

This table describes the delinquency notification process of Personnel Systems Management:

Description

1

Produces a delinquency report listing all Evaluation Confirmation forms not received and dated by the fifth of each month

2

Emails a delinquency report to department managers by the eighth day of each month

3

Produces a year-to-date delinquency frequency chart

4

Sends a delinquency report and delinquency frequency chart to the Administrator by the ninth day of each month
